home and community based services

Our approach focuses and builds upon each client’s strengths, interests, talents, preferences, and targets assistance to achieve specific results in someone’s life. We advocate for and support each client’s awareness and choice, while insuring their health and safety are maintained.

We currently support over 60 clients living in 18 apartments and community homes in Dubuque, Iowa, and the Iowa Quad Cities area.

Supported Community Living

Supported Community Living (SCL) provides 1 to 24 hours of skills training per day based on the consumer’s needs. The services is designed to assist individuals with disabilities in becoming more independent with daily living needs in a community setting.

Services may include, but are not limited to: personal needs, home skills, community skills, transportation and treatment services.

day habilitation

The Day Habilitation services provided through Hills & Dales assists and/or supports individuals in developing and/or maintaining life skills and community integration.

Day Habilitation is based at the Hills & Dales Community Center and is available from 10am - 2:30pm Monday through Friday.

A summer Day Habilitation program is also available.

supported employment services

Hills & Dales provides employment services by providing opportunities for individuals to explore integrated work environments and develop work opportunities to obtain employment, at or above minimum wage, related to their interest and skill ability.

We provide an individualized program of services focusing on:

  • Appropriate job skills

  • Work habits

  • Behaviors

  • Socialization

respite

Respite care services give temporary relief to the usual caregivers and provide all the necessary care the usual caregiver would provide during that time period.

home and vehicle modification

These services may provide for modifications to the home and/or vehicle that directly address the client’s medical health or are necessary to provide for the medical needs, welfare and safety of the individual and to increase or maintain independence.

Examples include:

  • Kitchen counters, sink space, etc.

  • Bathtubs and toilets to accommodate transfer

  • Grab bars and handrails

  • Ramps, lifts, and door, hall and window widening

  • Fire safety alarm equipment specific for disability

  • Specialized doorknobs and handles
